Submit this form to inform the Risk & Safety Officer of the need to add or remove a vehicle from the WWU auto insurance policy.

• The purchase of a vehicle by WWU
• The sale of a vehicle owned by WWU
• The donation of a vehicle to WWU
• The disposal of a vehicle owned by WWU

Walla Walla University is not responsible for the damage or other loss of personal property, including vehicles. Faculty, staff, and students are encouraged to carry their own insurance for all personal property they bring to WWU premises or on WWU-sponsored events.
